چکیده

A 65-year-old man was brought to the ER for altered mental status for 12 hours. At ER, the patient was lethargic with hyperthermia. The laboratory studies revealed intravascular hemolysis with anemia, hyperkalemia, and elevated LDH. The abdominal CT showed liver abscess with marked aerobilia. Respiratory condition deteriorated rapidly, necessitating intubation. A massive amount of blood exuded from the bronchial tube. Clostridium perfringens bacteremia was suspected from the blood smear report, which revealed spore-forming bacterias phagocytized by neutrophils with Giemsa stain (Picture 1), and gram positive rods by Gram stain (Picture 2). Clindamycin was added to meropenem and vancomycin, however, the patient died 6 hours after admission. Blood culture grew C. perfringens 1 day later. Acute life-threatening hemolysis is a characteristic complication of C. perfringens bacteremia, which is associated with a high fatality rate. Therefore, an early diagnosis and immediate onset of an appropriate antimicrobial therapy are crucial. Peripheral blood smear may be useful for the diagnosis of C. perfringens bacteremia.

Genotyping of Clostridium perfringens isolated from broiler meat in northeastern of Iran

Clostridium perfringens is an important cause of bacterial food poisoning worldwide. The disease is caused by C. perfringens enterotoxin (CPE) encoded by cpe gene. The aim of this research was to identify the different types of C. perfringens and the presence of cpe gene in isolated bacteria from broilers’ meat marketed in retail meat shops of Mashha...

Typing of Toxigenic Isolates of Clostridium perfringens by Multiplex PCR in Ostrich

Clostridium perfringens is an important pathogen that provokes numerous different diseases. This bacterium is classified into five various types, each of which capable of causing a distinct disease. There are various methods for the bacterial identification, many are labor-intensive, time-consuming, expensive and also show low sensitivity and specificity. The aim of this research was to identif...

Mitogenic activity of Clostridium perfringens enterotoxin in human peripheral lymphocytes.

Clostridium perfringens enterotoxin (CPE) was found to possess interferon (IFN)-producing and mitogenic activities to human peripheral blood mononuclear cells. Both activities were demonstrated only in the T lymphocyte-rich fraction from healthy volunteers. The IFN produced appeared to be gamma-type since the activity of the IFN was neutralized by antiserum against human IFN-gamma. With formali...

Clostn ' ch ' um perfringens Enterotoxl - is a Superantigen Reactive with Human T Cell Receptors V 136 . 9 and V 1322 By Paul Bowness , * Paul

Candidate superantigens were screened for their ability to induce lysis of human histocompatibility leukocyte antigen class II-positive targets by human CD8 + influenza-specific cytotoxic T cell (CTL) lines. Clostridium perfringens enterotoxin (CPET) induced major histocompatibility complex unrestricted killing by some but not all CTL lines. Using "anchored" polymerase chain reactions, CPET was...

Clostridium perfringens enterotoxin lacks superantigenic activity but induces an interleukin-6 response from human peripheral blood mononuclear cells.

We investigated the potential superantigenic properties of Clostridium perfringens enterotoxin (CPE) on human peripheral blood mononuclear cells (PBMC). In contrast to the findings of a previous report (P. Bowness, P. A. H. Moss, H. Tranter, J. I. Bell, and A. J. McMichael, J. Exp. Med. 176:893-896, 1992), two different, biologically active preparations of CPE had no mitogenic effects on PBMC. ...
